Policies

Card on File Policy

A valid credit card is needed on file to book any appointment. This credit card will only be charged if you arrive late to, no-show, cancel or reschedule your appointment.

Effective: January 1st, 2022

Deposit Policy

We require a deposit in order to book your appointment, which must be paid upfront and will go towards the final cost of your service. In the event that you cancel or reschedule your appointment less than 24 hours in advance your deposit will not be refunded under any circumstance. All services rendered are non-refundable and non-transferrable.

Effective: March 1st, 2021

Cancellation Policy

Failure to cancel your appointment 24 hours in advance will result in a minimum charge of $15.00 or 50% of your service cost, whichever is greater. This charge will be henceforth known as a Cancellation Fee and will not be refunded under any circumstance.

Effective: January 1st, 2022

Subscription Cancellation Policy

Member will be issued a full refund if plan is cancelled within 3 days of original purchase. We have a 30-day cancellation notice policy prior to the next billing date. If you wish to not be billed in a certain month then your membership MUST be cancelled 30 days prior to that bill date (date of purchase).

Rescheduling Policy

Failure to reschedule your appointment 24 hours in advance will result in a minimum charge of $15.00 or 25% of your service cost, whichever is greater. This charge will be henceforth known as a Deposit and will not be refunded under any circumstance. However, in this case the deposit can be rolled over and applied toward the final cost of your next appointment.
Failure to reschedule your appointment 24 hours in advance a 2nd time will result in a minimum charge of $15.00 or 30% of your service cost, whichever is greater. This charge will henceforth be known as a Rescheduling Fee which cannot be rolled over as a deposit for your next appointment and will not be refunded under any circumstance.
Failure to reschedule your appointment 24 hours in advance a 3rd time will result in an additional charge of $15.00 or 50% of your service cost, whichever is greater. This charge will henceforth be known as the 3rd Strike Fee and will be charged immediately at appointment booking.
Failure to notify REJUVENATION slim body works staff at least 24 hours in advance a 4th time will result in termination of your privileges at REJUVENATION slim body works, Medical Spa, Midland, Tx.

Credit Card Fees Policy

All credit card purchases will incur a 3.99% CC processing fee. Cash payments will not incur any fees and are highly encouraged.

Late Arrival Policy

Failure to arrive at your appointment within the 15-minute courtesy window will result in an immediate charge of $15.00. This charge will henceforth be known as a Late Arrival Fee and will not be refunded under any circumstance.
Arrival at your appointment 16 or more minutes late will result in a No-Show Fee which will not be refunded under any circumstance.

Complimentary Service Policy

Rescheduling an a appointment for any complimentary service is not allowed, due to increased volume of no-shows, same day rescheduling and cancellation requests. Complimentary services will be considered redeemed at time of booking.

Effective: April 29th, 2022

Consultation Fee Policy

All appointments which require an annual consultation with the physician will incur a consultation fee.

- Neurotoxin/Dermal Filler Treatments

- ALL Laser/Light Treatments

- Body Slimming

- RF Skin Tightening/Body Contouring
